Help Shape Our Community’s Health

Your opinion about the health in our Sioux Empire is important, so we are asking for your help. The Sioux Falls Health Department, Avera McKennan Hospital and University Health Center and Sanford USD Medical Center are conducting a Community Health Needs Assessment. This process assists community leaders in understanding the health status of the area and identifying priority health issues affecting residents.

As part of the community assessment, we partnered with the Center for Social Research at North Dakota State University to conduct a survey of the Sioux Empire. By participating in this Community Health Needs Assessment survey, you can help shape the future health, well-being, and quality of life of the residents in the area. The last part of the survey asks for information about you and will help us learn more about our community. This information will be kept confidential.

This survey is voluntary. All answers to the questions are strictly confidential, and no identifying information will ever be linked to any of the responses. Responses will be used to develop community-wide strategies to help residents live healthier lives. Please take a few minutes of your time now to complete this online survey. It should take you no more than 20 minutes to complete the survey.
