Preliminary Health Data About Our Community

The Sioux Falls Health Department and community partners have been working on a Community Health Assessment over the past year, exploring data about individual and community health outcomes and about what impacts our health. Prelminary data is now available for review. The partners will be finalizing this information in the coming weeks and will be developing a collaborative Community Health Improvement Plan (CHIP) with strategies designed to help our residents Live Well.

More information will be coming in the near future with the finalized data report and CHIP plan, so stay tuned!
